You may receive a vision screening exam once every twelve months. This includes a brief history, a visual field and glaucoma screening, and refraction for eyeglasses. You can receive prescription eyewear (glasses and/or contact lenses) once every two years at no cost up to a $200 plan benefit limit. Amounts over $200 are the member's responsibility.
You do not need to contact CommunityCare or your PCP before scheduling your routine vision screening; authorization rules may apply. You will be charged your office co-payment for the screening. Remember to show your ID card at your office visit.
If your vision provider finds a potential medical problem with your eyes, you will need to contact your PCP for further treatment options.
