What is CommunityCare Prescription Drug Plan?
CommunityCare Prescription Drug Plan was created especially for people on Medicare. CommunityCare has contracted with the federal government to operate CommunityCare Prescription Drug Plan.

Part D Coverage only, with no medical benefits and a low monthly premium of $20.80
Do I lose my Medicare when I join?
No. You must be entitled to Medicare Part A and/or enrolled in Medicare Part B in order to join our plan. While you are a member of this plan, you will show your CommunityCare Prescription Drug Plan ID card at the pharmacy.
When can I enroll?
Most people are eligible to enroll in Medicare when they turn 65, although some people are eligible before age 65 due to a disability. You can submit an application for our plan beginning 3 months prior to your Medicare A and/or B effective date and your plan will begin on the same day as your Medicare.
There may be other times when you can change your plan - for example, if you move out of your plan's service area, you would have a special election period to choose a plan in your new area.
You can also change plans each year during the Annual Election Period. The Annual Election Period is October 15 through December 7 - any plan changes you make during this time go into effect January 1.

What about medical benefits?
A Part D plan like CommunityCare Prescription Drug Plan covers outpatient prescription drugs. Your medical services would be covered by Original Medicare.
What are my disenrollment rights and responsibilities?
You may end your membership in our plan only during certain times of the year, known as enrollment periods. All members have the opportunity to leave the plan during the Annual Enrollment Period and during the Medicare Advantage Disenrollment Period. In certain situations, you may also be eligible to leave the plan at other times of the year. Until your membership ends, you must keep getting your drugs through our plan. Although Medicare is a Federal program, CommunityCare Prescription Drug Plan is available only to individuals who live in our plan service area. To stay a member of our plan, you must keep living in this service area.
