Senior Center

At CommunityCare's Senior Center, you can:

  • Learn more about Oklahoma Dual Complete – Come to one of our seminars or visit with a licensed representative one-on-one.
  • Receive help with enrollment – One of our licensed team members are available to assist you.
  • Receive customer service – We have Member Advocates here to assist you with benefits, premiums and claims questions.
  • Members can join us for exercise groups, health classes, movie days and more at our Senior Center. Check the class schedule for details & upcoming events.
  • No appointments necessary Monday through Thursday- you are welcome to drop in anytime!

Senior Health Plan is an HMO plan and a D-SNP plan with a Medicare & Medicaid contract. Enrollment in Senior Health Plan depends on contract renewal.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact www.medicare.gov or 1-800-MEDICARE (1-800-633-4227) to get information on all of your options. Medicare customer service representatives are available 24 hours a day, 7 days a week. TTY/TDD users should call 1-877-486-2048.
