Remember that your clients that you enroll by phone or fax will NOT show in your SunFire portal.

Agents can email applications and SOAs to medicaremarketing@ccok.com. If you do not have secure email, please fax your application to 918-878-5978. Please include the application, a Scope of Appointment and any comments/questions you may have.

Senior Health Plan is an HMO plan and a D-SNP plan with a Medicare & Medicaid contract. Enrollment in Senior Health Plan depends on contract renewal.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area.
